When we become systematic it ceases to be about people. Blame is symptomatic of an a-systemic approach.
Trace the problem back at least four steps. Where we choose to stop looking for a blame is subjective.

An effective organization is, well, organized and therefore is not particularly vulnerable to the personalities and deficiencies of individuals.
Let's put it yet another way. Does your organization rely often on heroic effort to achieve goals? Then it is vulnerable to villainy as well.
